Patient data was analysed using non-parametric statistics with Mann-Whitney test.</p> <p><italic><bold>RESULTS:</bold> </italic>In total there were 328 patients, aged between 2 months and 17 years, with 402 groin hernia defects including metachronous and rare defects. The herniorrhaphy was performed for indirect hernia defects (n=186) and herniotomy — for indirect (<italic>n</italic>=206) as well as for direct (<italic>n</italic>=6) and femoral (<italic>n</italic>=2) defects. Technically, there were no intraoperative complications in any case in either series. Conversion to open procedure was required in one patient of 1st series (0,5%) with giant hernia because of the impossibility to maintain due pneumoperitoneum. The postoperative recovery was prompt and uneventful with restoration of mobility and oral intake within 3–6 hours in all patients with no difference between the series. Adverse postoperative events were noted in 10 patients after herniorrhaphy — hydrocele (<italic>n</italic>=6; 3.2%) and hernia recurrence (<italic>n</italic>=4; 2.2%) whereas after herniotomy there was only one case (<italic>n</italic>=1; 0.5%) of hydrocele and none of recurrence. The overall rate of these complications was significantly lower in the herniotomy series vs herniorrhaphy (<italic>p</italic>=0.004).</p> <p><italic><bold>CONCLUSION:</bold> </italic>Laparoscopic intracorporeal techniques both herniorrhaphy and true herniotomy are safe and effective for pediatric hernia repair. True herniotomy appears to be a more robust technique to minimise incidence of postoperative hydrocele and recurrence.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование. </bold>Оптимальная техника лапароскопических операций корригирования паховой грыжи у детей остаётся предметом дискуссий и предложений по дальнейшему совершенствованию.</p> <p><bold>Цель.</bold> В настоящей публикации мы представляем собственные результаты лапароскопических операций герниорафии и герниотомии и приводим сравнительную оценку их надёжности.</p> <p><bold>Методы.</bold> Проведен ретроспективный анализ данных пациентов с диагнозом «паховая грыжа», у которых были выполнены лапароскопические операции с использованием интракорпоральной техники — герниорафия (1-я серия) и герниотомия (2-я серия). Статистический анализ данных проводился с использованием U-теста Манна–Уитни и критерия χ<sup>2</sup>.</p> <p><bold>Результаты. </bold>Всего у 328 пациентов в возрасте от 2 мес. до 17 лет выполнено закрытие или полное устранение 402 грыжевых дефектов, включая метахронные и редкие типы дефектов. Герниорафия была выполнена при закрытии непрямых дефектов (<italic>n</italic>=186), гернитомия — для устранения непрямых (<italic>n</italic>=208), а также прямых (<italic>n</italic>=6) и феморальных дефектов (<italic>n</italic>=2). Технически интраоперационных осложнений не было ни в одном случае обеих серий. Конверсия потребовалась в 1-й серии у одного пациента с гигантской пахово-мошоночной грыжей (0,3%), по причине невозможности создания надлежащего пневмоперитонеума. Послеоперационный период протекал благоприятно у всех пациентов обеих серий, с восстановлением двигательной активности и возобновлением орального кормления в течение 3–6 ч, независимо от используемой хирургической техники. Неблагоприятные послеоперационные эффекты отмечены у 10 пациентов 1-й серии — гидроцеле (<italic>n</italic>=6; 3,2%) и рецидивы грыжи (<italic>n</italic>=4; 2,2%), во второй серии рецидивов не было, гидроцеле наблюдалось у одного пациента (<italic>n</italic>=1; 0,5%). Суммарная частота неблагополучных результатов после гернитомии была статистически ниже, чем после герниорафии (<italic>р</italic>=0,004).</p> <p><bold>Заключение. </bold>Лапароскопические операции герниорафии и герниотомии с интракорпоральной техникой безопасны и эффективны для лечения паховой грыжи у детей.
