The obtained findings were compared with intraoperative findings.</p><p><bold>Results.</bold> Complete and deep impairments of median nerve conduction were revealed after supra-, trans-condylar fractures of the humerus and dislocation in the elbow joint many weeks and months later after the date of injury. 6 out of 10 children were operated on because of the lack dynamics in neurological symptoms. At the same time, in two cases ultrasound examination showed that the nerve trunk was completely involved in the callus and strangled; this finding corrected surgical tactics. Intraoperatively , three variants of median nerve injury were described: (1) the nerve has an angular deformation, is surrounded by bone or scar tissues, compressed and fixed to the underlying bone or soft tissues without nerve bundles damage; (2) in addition to the abovementioned, the nerve has damaged bundles with various degrees of damage; (3) the nerve is located in scars in its usual position with intact bundles and without a distinct neuroma formation.</p><p><bold>Conclusion.</bold> Comparison of clinical, edectroneuromyographic and ultrasound findings plus intraoperatively identified variants of nerve damage allows to choose a proper surgical tactics - decompression and neurolysis with restoration of the nerve natural position in tissues, microsurgical partial autoplasty or microsurgical suture of the nerve trunk. Ultrasound examination helps to clarify indications for surgery.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Цель исследования</bold> – показать варианты закрытых повреждений срединного нерва (СН) у детей, виды хирургического лечения, обусловленные типом повреждения СН, связь их с данными дооперационных клинического, электромиографического и ультразвукового исследований.</p><p><bold>Материал и методы. </bold>Обследованы 10 детей с применением балльной шкалы, электронейромиографии, ультразвукового метода. Полученные данные сопоставлены с интраоперационо обнаруженными повреждениями срединного нерва при его закрытой травме.</p><p><bold>Результаты.</bold> Выявлены синдромы полного и глубокого нарушения проводимости срединного нерва после над-, чрезмыщлкового перелома плечевой кости, вывиха в локтевом суставе по прошествии многих недель и месяцев со дня травмы, потребовавшие у 6 из 10 детей хирургического лечения в связи с отсутствием положительной динамики неврологических симптомов. При этом ультразвуковое исследование (УЗИ) нерва в двух случаях обнаружило, что нервный ствол полностью вовлечён в костную мозоль, сдавлен, это уточнило показания к операции. Интраоперационно оказалось возможным выделить 3 варианта изменений срединного нерва: 1-й вариант, при котором нерв имеет угловую деформацию, окружён костной или рубцовой тканью, сдавлен и фиксирован к подлежащей кости или мягким тканям без повреждения пучков нерва; 2-й вариант, при котором, помимо выше отмеченного, нерв имеет повреждённые в той или иной степени пучки; 3-й вариант, при котором нерв находится в рубцах в его обычном положении без повреждения пучков и образования отчётливой невромы.</p><p><bold>Заключение.</bold> Сопоставление клинических, электронейромиографических данных и данных УЗИ с интраоперационно выявленными вариантами повреждения нерва определяет метод хирургической операции – это декомпрессия и невролиз с восстановлением естественного положения нерва в тканях, или микрохирургичекая частичная аутопластика, или микрохирургический шов нервного ствола.
