Of these, 32% have dysfunctional urination, which means difficult urination due to the inability of external urethral sphincter and pelvic floor muscles to relax properly. Consequently, such abnormality can be accompanied by urinary incontinence and infectious complications. Timely diagnostics and proper understanding of the pelvic floor role in urination are essential for the effective treatment.</p> <p><bold>AIM: </bold>To improve treatment outcomes in children with pelvic evacuation dysfunctions through the implementation of transperineal sonography.</p> <p><bold>METHODS:</bold> The authors present their observational, single-center, and randomized trial. The main sample included 103 children with pelvic evacuation disorders who were consulted at N.F. Filatov Children's City Hospital in Moscow from 2018 to 2025. All children underwent transperineal sonography to examine their pelvic floor, triple uroflowmetry with assessment of the residual urine, and electromyography of the perineal muscles.</p> <p><bold>RESULTS:</bold> Children were divided into two groups by gender (58 girls and 45 boys). According to the findings obtained at three uroflowmetry examinations, urinary flow rate was decreased by 27%, in average, in boys, while in girls this parameter was at the lower norm limit. Intermittent or staccato urination was seen in all children in the studied group. An electromyography curve during urination showed the increased pelvic floor muscle activity. Moreover, ultrasound examination revealed the residual urine (from 10 to 30%) in the bladder of 53 children. Transperineal sonography in children with dysfunctional voiding has found a specific feature: virtually, no displacement of the urethra and bladder neck during functional tests, which was interpreted as manifestations of the paradoxic pelvic floor muscle activity. The increased puborectal loop size, anal canal width, and the decreased size of anorectal angle indicate anal sphincter hypercontractility. These findings correlated with transperineal sonography findings.</p> <p><bold>CONCLUSION:</bold> Transperineal sonography is a simple, available and non-invasive technique which demonstrates good diagnostic values in identifying dysfunctional voiding in children.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование.</bold> Распространённость эвакуаторной дисфункции органов малого таза составляет 17–22% всех урологических обращений в детской популяции, из них до 32% случаев заболевание имеет функциональный характер и проявляется затруднениями во время микции из-за неспособности наружного сфинктера уретры и мышц тазового дна расслабиться должным образом, что может сопровождаться недержанием мочи, присоединением инфекционных осложнений. Своевременная диагностика и понимание роли тазового дна в акте мочеиспускания важны для эффективного лечения.</p> <p><bold>Цель исследования</bold> ― продемонстрировать улучшение результатов диагностики эвакуаторной дисфункции органов малого таза у детей за счёт внедрения трансперинеальной сонографии.</p> <p><bold>Методы.</bold> В основную выборку обсервационного одноцентрового аналитического исследования с нерандомизированной группой сравнения включено 103 ребёнка в возрасте 5–18 лет с эвакуаторными нарушениями тазовых органов, которые в период с 2018 по 2025 год наблюдались в ДГКБ им. Н.Ф. Филатова. Всем детям проводили трансперинеальную сонографию для оценки состояния тазового дна, трёхкратную урофлоуметрию с определением остаточной мочи и электромиографию мышц промежности.</p> <p><bold>Результаты.</bold> В зависимости от пола дети были разделены на две группы (58 девочек и 45 мальчиков). По данным трёхкратной урофлоуметрии в группе мальчиков выявлено снижение средней скорости мочеиспускания на 27%, у девочек показатели данного параметра находились на нижней границе нормы. У всех детей, вошедших в исследование, выявлено прерывистое мочеиспускание или мочеиспускание серией частых коротких толчков (тип «стаккато»). На кривых электромиограмм во время мочеиспускания отмечалось повышение активности мышц тазового дна; по данным ультразвукового исследования в 53 случаях в мочевом пузыре имелась остаточная моча (от 10 до 30%). У детей с дисфункциональным мочеиспусканием при трансперинеальной сонографии выявлена характерная особенность ― практически полное отсутствие смещения уретры и шейки мочевого пузыря при выполнении функциональных проб, что расценивалось как проявление парадоксальной активности мышц тазового дна. Увеличенные размеры пуборектальной петли, ширины анального канала и уменьшение размеров аноректального угла свидетельствуют о гиперконтрактильности анального сфинктера и коррелируют с результатами трансперинеальной сонографии.</p> <p><bold>Заключение.</bold> Трансперинеальная сонография представляет собой простой, доступный, неинвазивный метод и демонстрирует хорошую диагностическую значимость в выявлении дисфункции органов малого таза у детей.</p></trans-abstract><kwd-group xml:lang="en"><kwd>urinary bladder neurogenic dysfunction</kwd><kwd>dysfunctional voiding, children</kwd><kwd>urodynamics</kwd><kwd>transperineal ultrasonography</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>нейрогенная дисфункция мочевого пузыря</kwd><kwd>дисфункциональное мочеиспускание у детей</kwd><kwd>уродинамическое исследование</kwd><kwd>трансперинеальная сонография</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Sureshkumar P, Jones M, Cumming R, Craig J.
