In 80% of cases (12 patients), the botulinum toxin injection procedure was performed more than once (2 children ― three times) with an interval of 6 months. The main complaints were constipation in all cases, colostomy in 60% of cases, bloating and flatulence in 80%, recurrent Hirschsprung-associated enterocolitis in 80%. The average maximum size of the colon according to irrigography was 80 mm.</p> <p><bold>RESULTS:</bold> The analysis showed that the dynamics after botulinum toxin administration showed statistically significant changes in the occurrence of constipation (<italic>p</italic> &lt; 0.001) and fecal incontinence (<italic>p</italic> = 0.006) and the frequency of Hirschsprung-associated enterocolitis (<italic>p</italic> = 0.001). During the analysis of pressure changes in the anal canal before and after administration of Botox, statistically significant changes in pressure values were observed at rest, with voluntary contraction and with coughing (<italic>p</italic> &lt; 0.001).</p> <p><bold>CONCLUSION:</bold> The study shows that the administration of botulinum toxin in anal sphincter achalasia in patients undergoing Hirschsprung's disease surgery significantly facilitates the manifestations of obstructive colopathy.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование.</bold> В статье представлен опыт лечения пациентов, прооперированных по поводу болезни Гиршпрунга и страдающих обструктивной колопатией, обусловленной ахалазией внутреннего анального сфинктера.</p> <p><bold>Цель исследования</bold> ― обоснование тактики лечения обструктивной колопатии, обусловленной гипертонусом внутреннего анального сфинктера после операции «низведения» по поводу болезни Гиршпрунга, путём инъекций ботулинического токсина.</p> <p><bold>Методы.</bold> В исследование включено 15 пациентов, из них 9 мальчиков, средний возраст на момент процедуры 4 года. В 80% случаев (<italic>n</italic> = 12) процедура введения ботулотоксина проведена более одного раза (2 детям ― трижды) с интервалом 6 месяцев. Основными жалобами были запоры (во всех случаях), каломазание (60%), вздутие живота и метеоризм (80%), рецидивирующий Гиршпрунг-ассоциированный энтероколит (80%). Средние максимальные размеры толстой кишки по данным ирригографии составили 80 мм.</p> <p><bold>Результаты.</bold> В динамике после введения ботулотоксина отмечались статистически значимые изменения частоты встречаемости запоров (<italic>p</italic> &lt; 0,001), недержания кала (<italic>p</italic> = 0,006) и Гиршпрунг-ассоциированного энтероколита (<italic>p</italic> = 0,001).
