BACKGROUND: Presentation of clinical case data will help improve diagnostics and management, as well as tactics of surgical treatment of adolescent girls with torsion of the uterine appendages.

CLINICAL CASE DESCRIPTION: The article presents two cases of uterine appendage torsion in adolescent girls with paraovarian cysts, initially hospitalized with acute abdomen and symptoms nonspecific for torsion. Instrumental diagnostic methods (ultrasound and MRI of the pelvic organs) revealed the presence of cysts, but before diagnostic laparoscopy, torsion was unknown in both cases. Organ-preserving tactics were chosen for surgical treatment.

CONCLUSION: Based on the presented clinical observations, conclusions were made about the importance of timely diagnostics in the observation and treatment of girls with acute abdomen. When excluding the diagnosis of "acute appendicitis" in girls, it is necessary to exclude acute gynecological pathology.
