UNIFIED CLASSIFICATION OF PERITONITIS IN CHILDREN

The team of authors of the article, based on many years of experience in the diagnosis and treatment of peritonitis in children, after studying domestic and foreign literary sources, came to the conclusion that there is still no unified classification of peritonitis in terms of polyethological disease. As indicated by all researchers dealing with this problem. In this regard, the authors decided to compile a unified classification of peritonitis in children, which takes into account the existing classifications of the disease and all its forms of manifestation and complications. The proposed classification consists of 12 headings: by course, by origin (by nature), by the nature of the pathological process, by the path of infection into the abdominal cavity, by the microbial factor, by the nature of exudate and contents of the abdominal cavity, by prevalence, by the stage of development of the purulent-septic process, by the nature of the purulent process, by complications, according to the outcome of the disease. According to the course of peritonitis, it was divided into acute and chronic, by origin into primary, secondary and tertiary. In the category of tertiary peritonitis, various forms of its manifestation were indicated as continuing peritonitis, possibly by the type of progression or a sluggish pathological process that depends on reactivity, the presence of concomitant pathology and the immune state of the children's body.
According to the prevalence, the division of peritonitis into local and widespread is maintained. In the section of local peritonitis, limited peritonitis includes abscess types 1, 2, 3 and infiltrate. In the heading the nature of the course of the pathological process, the authors agree with the indication of free, partially abscessed, totally abscessed, infiltrative (loose, dense) and mixed.
In the category of the outcome of the disease, recovery and death are distinguished. In turn, recovery is assessed according to a three-point system: complete recovery - with a good outcome, satisfactory and unsatisfactory results, which reflects the degree of effective treatment of sick children with peritonitis.
The proposed unified classification of peritonitis in children is being discussed. At the same time, we will be waiting for the opinion and suggestions from all pediatric surgeons to improve it.
